Client confidential case history intake form and consent

The therapist does not diagnose or prescribe, (order for use of medicine).

There is a confidential case history intake form to fill for new clients which includes a HIPAA consent form. This is sent with the first booking confirmation.

Payment and cancellation Policy

Payment: For in person sessions, payment is due after each session. For online bookings, payment is due upon scheduling. For 3 hrs to 5 hrs day bookings, a deposit is required and non refundable. 

 

Cancelation: Please respectfully give a minimum of 24 hrs notice to cancel an appointment and 48 hrs for 3 hrs to 5 hrs day bookings. Otherwise there will be a 50% charge of last minute cancelation fee.
